Overview
These Terms of Use apply to your use of the Country Plaza Medical Centre website and the services we offer through it. By using this website, you agree to these terms. If you do not agree, please do not use the website.
The information on this website is for general information only. It is not medical advice and is not a substitute for an assessment, diagnosis or treatment by a qualified health care provider.
Changes to these terms
We may update these terms to reflect changes in the law, our services or our practices. Updates are posted on this page with their effective date. If you keep using the website after an update, you accept the updated terms.
We may add, remove or change content on the website at any time. We may correct errors or omissions, but we are not obliged to.
Medical information and emergencies
We do not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
Please do not use our website forms or email for urgent medical concerns. The health articles on our blog are general education and may not apply to your situation; please speak to your physician about your own health.
Age, eligibility and dependants
If you are under 18, the age of majority in Alberta, a parent or legal guardian should use our online services and make bookings for you. You may book for, or contact us about, a dependant such as your child or a person for whom you hold a power of attorney, if you have the legal authority to do so. You are responsible for making sure the information you give us about them is accurate.
Appointments and bookings
You can request an appointment by phone, in person or online. Online booking and the Ava Connect patient portal are provided by a third-party service, and its own terms and privacy policy apply when you use it.
We cannot guarantee that every appointment request will be accepted, and in unexpected circumstances we may need to reschedule or cancel a confirmed appointment. Our cancellation, no-show fee and other clinic rules are set out in our Clinic Policies.
Our health care providers
Care at Country Plaza Medical Centre is provided by regulated health professionals, who are responsible for the care they give and follow the standards of their regulatory college. A provider may decide that a service is not appropriate for you and may decline to provide it.
Prescriptions, notes and other documents we provide are for your personal use only and must not be shared, altered or used for any other purpose.
Third-party services and websites
Our website links to services and websites run by others, such as our online booking provider, Google Maps, and government and health organizations. We do not control them and are not responsible for their content, availability or practices, and a link does not mean we endorse them. Your use of them is at your own risk and subject to their own terms and privacy policies. If you use a service from a separate business, you deal directly with that business.
Ownership of website content
The text, images, logos, graphics and other material on this website belong to Country Plaza Medical Centre or are used under licence, and are protected by copyright and other laws. You may view and print pages for your own personal, non-commercial use. You may not copy, reproduce, modify, publish or distribute any of it without our written permission.
Using our website responsibly
You are responsible for anything you send us through this website, our forms, by email or on our social media channels. Please be careful about sharing medical details online: what is posted online can stay there for years. You agree not to:
- Send content that is false, misleading, unlawful, threatening, abusive, hateful, harassing, defamatory, obscene or harmful to minors
- Pretend to be another person, including one of our physicians or staff, or use someone else’s information without permission
- Send spam, chain letters or unsolicited advertising
- Infringe anyone’s copyright, trademark, privacy or other rights
- Try to gain unauthorized access to the website, its systems or other people’s information
- Disrupt or interfere with the website, its servers or networks, or introduce viruses or other harmful code
- Use bots, scrapers or other automated tools to copy content or collect data from the website
We may remove content, or block access to the website, for anyone who does not follow these terms. We may keep or disclose information when the law requires it or to protect the rights and safety of our patients, staff and the public.
Privacy
We respect your privacy. Please read our Privacy Policy to learn how we collect, use, share and protect your personal and health information.
Disclaimers and limitation of liability
We work to keep the information on this website accurate and up to date, but we cannot guarantee that it is complete, current or free of errors, or that the website will always be available or free of viruses. The website is provided “as is”, and you use it at your own risk.
To the extent permitted by law, Country Plaza Medical Centre and its physicians and staff are not liable for any loss or damage arising from your use of, or reliance on, this website or any website linked to it.
Indemnity
You agree to indemnify Country Plaza Medical Centre against any claims or losses arising from your misuse of this website or your breach of these terms.
Governing law
These terms are governed by the laws of the Province of Alberta and the federal laws of Canada that apply there. The courts of Alberta have exclusive jurisdiction over any dispute about these terms or this website.
Events beyond our control
We are not responsible for any delay or failure caused by events beyond our reasonable control, such as severe weather, fire, flood, a pandemic, power or internet outages, equipment or software failure, or government action.
Severability
If any part of these terms is found to be invalid or unenforceable, that part will be removed and the rest will remain in effect.
